Clinical Practice Closing 9/30/22
I am announcing that I will be closing my medical practice effective September 30, 2022. I will continue to remain active as a consultant and student mentor.
After 46 years in clinical practice I have decided to redirect my efforts in my personal and professional life. It has been a great pleasure meeting and caring for my patients. I wish all of you good health and all the best in the coming years.
You can arrange the care of another physician of your choice by contacting the American Podiatric Medical Association at www.apma.org for a list of podiatric physicians/surgeons practicing in your local region or state. You may also contact the physician referral line at your local hospital.
REQUESTS FOR MEDICAL RECORDS
Your medical records are confidential and will remain on file under our custodianship according to state and federal statues. Please note, by law we cannot share your medical information without your written consent. Should you need a copy of your medical records you will need to sign an authorization form for their release. You may sign this form at our office or fax a medical records release to 219.769.3880 before September 30, 2022 or you may complete the necessary paperwork at your first visit with your new doctor and email to amarasco@hotmail.com.
After September 30, 2022, you will need to Submit a Request for Release of Medical Records through this website. After submitting, open the attached Patient Authorization for Release of Medical Records Form. Print, complete, sign and date the document and email to amarasco@hotmail.com.
